A rheumatic disease suspected?

I so, an exact examination is recommended, to differentiate between  inflammatory and non-inflammatory symptoms, together with lab and imaging data to make a good diagnosis.

It is important to examine already a single swollen joint, to prevent longstanding pain and even bone destruction.

Consider a rapid rheumatic assessment also in case of systemic symptoms like undefined fever, weight loss and night sweating - after exclusion of infection or a malignant disease!

After age of 50 years, even temporal headache, visual disturbances and jaw claudicatio have to be considered as signs of a rheumatic disease and urgently need an acute rheumatic assessment to avoid visual loss and other complications.  

Rheumatic diagnosis already assessed?

There are about 200-400 rheumatic diseases, together with more than 5000 rare genetic diseases, also affecting the musculoskeletal system. Gratulations, if you already know your diagnosis!

A rheumatologist typically treats inflammatory diseases of the joints, m muscles and the spine like rheumatoid arthritis, polymyalgia rheumatica, spondyloarthritis (ankylosing spondylitis) with psoriatic arthritis, gout and pseudogout. Besides, the rheumatologist treats vasculitides like systemic lupus erythematosus, giant cell arteritis, polyangitis (granulomatous, formerly named as Morbus Wegener; eosinophilic, formerly named as Churg-Strauss syndrome), Behcet's syndrome and other systemic diseases like systemic sclerosis.

Fibromyalgia is well-defined as a typical non-inflammatory rheumatic disease

What to consider during therapy?

Tell your physician all symptoms, reports, allergies, intolerances and diagnoses before starting any treatment!

Specialists for internal medicine, orthopedic surgery or physical medicine will treat you. Sometimes, interdisciplinarity is needed with specialists for dermatology, ophthalmology, pulmology, ENT, cardiology, gastroenterology or urology.

Today we have many good therapies, both non-pharmaceutical and pharmaceutical. An exact diagnosis and understanding of all symptoms is necessary to avoid "non-responders".

As rheumatic diseases are often chronic, patients should be educated and have rapid access to a rheumatologist if needed for acute situations.

• Diplomas of the Austrian Medical Association in emergency medicine 1995, in manual therapy 1995 and as clinical investigator 1999
• 1995 - 2023 Specialist in internal medicine for rheumatic patients at the department of internal medicine at the university of Innsbruck, later called the Medical University of Innsbruck (with interruptions)
• 1997-1998 Research Fellow at the division of rheumatology in the MAYO-clinic, Rochester (MN, USA) 
• 2000 Habilitation in internal medicine (with focus on rheumatoid arthritis)
• 2006 - 2009 Head of the department for internal Medicine and the devision of geriatrics at the general public hospital of the Elisabethinen in Klagenfurt
• 2000-2023 Associate Professor, since then retired
• Private office since 2024 

• President of the International Society for Behcet's disease
• Member of the MAYO Alumni Association and the Philip S. Hench Society, Rochester (MN, USA)
